This section provides an overview of the 9080-3 Establishment Application for Export and also step-by-step instructions on how to approve or reject the application in PHIS. As you receive these applications in your establishments, bear in mind two very important points about the 9080-3:
(a) Not every importing country requires exporters to submit a Form 9080-3 to certify eligibility. Check the Export Library to see whether or not it is required.
(b) If it is required, and the establishment submits a 9080-3 for approval, carefully check to ensure the exporter has completed the form accurately, including attaching any required supplemental documents. These applications are routed all the way up to FSIS Headquarters for final approval, and incorrect applications may cause unnecessary delays in certifying eligibility.
